If you would like to file a claim against the City for an incident / damage that you believe was caused by the City of Norwalk, please use the following form.  The form is fillable online, but needs to be printed, signed and delivered to the City Clerk's Department either in person or by U.S. Postal Service as an original signature is required.  Filing a claim does not guarantee reimbursement.  Each claim is investigated by a third party claims administrator.
